The human body is constructed in the way that if there is an injury the blood flows out of the body to prevent any foreign substance to enter the blood circuit, however that isn't always successful, in particular when there where pathogenic substances directly on the object that caused the injury. So yes there is always the danger of disease transmission. That's why fighters have to test on HIV and Hepatitis C regularly. It doesn't even have to be any serious disease, any blood and sweat sweat from the opponent entering the injury can cause nasty inflammations.
