Really interesting question actually. My guess would be that since there are a number of techniques you have to master before you get your next belt, they awarded themselves a new belt (although perhaps just not one member, but the whole family in agreement)after they had mastered a certain numbers of new techniques.
But that's just a guess. But I'm pretty certain that more than one family member had to agree that another member deserved a higher rank than anyone was allowed to hand out.
