As for SelfDefense trying to make out Europe is such a peaceful loving community. Erm..not quite. There are street fights going on every day in every major town or city here in England. And I bet its a similar story in alot of countires in Europe, specifically Eastern Bloc countries. Street fights here are genrally nothing to fear. The many I've got involved in, intentionally or not, are usually quite honorable affairs where there are handshakes after and nobody dies.
Think it was Charles the 2nd, he once demanded a boxing ring by assembeled in London so if any men have disagreements they can go there to sort it. I like.

You are assuming Eastern + England. That is not the country I was talking about. =) Indeed street fights are nothing to fear, usually. Perhaps in America they are c-r-a-z-y (well, obviously because they have guns).

Yes, they are, no matter in what country you get attacked. That's because you never know what the attacker does to you when you get KOed. And you never know whether you fall unlucky when the attacker just pushes you, so that you hit your head on concrete.
Therefore, talking about self defense and pride aside, every fight should be avoided and every attack has to be stopped before you get hurt. Again, I'm not talking about revenge and I'm not talking about over-reacting, but you have to do what it at least necessary - which depends on the situation and circumstances. If there is an attacker with a knife, I wouldn't recommend counting on to keep the margin between "what is necessary to stop the attack" and "getting hurt" too small as you don't know what the attacker is able to do.

Voiceless, indeed might fall unlucky. Though, what about the backwards roll that they teach you in Judo, or the fall break (smack hands on floor 45°) for that matter? Not to mention that a self defense practitioner should always keep in mind to keep his head tucked in to avoid it to slam on concrete and get a whiplash? Of course I understand if really surprised and not that trained, one would not remember that reflex. I suppose one has to train these reflexes over and over frequently.
A guy in my club is very flexible on that ground. If he gets pushed unexpectedly he immediately falls to the floor and does his roll. Don't know if that would work for real though. Concrete is not that soft.
I have asked some people about 'intentionally wanting to kill someone' and they said that there has to be something wrong in your mind to want to do such a thing. I don't think killing someone is necessary even if that person is trying to kill you. A person does not only stop when killed. If a person is unconscious, that is sufficient. I think this is mostly the reason why military defense guys train to beat the crap out of the other guy asap and then restrain him or something else. Killing? Maybe in a war.
Killing is just inhumane.
//Edit: I have concluded that there are no one-answer solutions to getting out of a specific fight situation. There are too many factors that play a role - location, surroundings, situation, etc. So yeah in the end you do what you can do to end the fight as quickly as possible.
