Okay first off, your left hand when kicking is wandering, you need to keep it secure next to your temple. I noticed that straight away with your punches as well, you should start doing bigger combos on the bag without all the pauses, this will not only increase your combo ability but your cardio too.
For example, try adding a right hook, followed by a left elbow to it. Therefore, it'd be Jab - Cross - Right Kick - Jab - Cross - Left Kick - Jab - Cross - Right Hook - Left Elbow
On the pad work, try and respond quicker. You can't think in a fight, you have to know how to do it off by heart, and working on response will help you a lot. Try getting a few combos worked out in your head which you can pull off without even telling your body to, it does it automatically.
