It could be a number of things Im pretty sure its due to improper technique. What I beleive is happening is your wrist is buckeling when you hit. Do you feel your wrists bend when you hit the bag? Bend forward or back, if so thats whats causing your pain and its because your not keeping your hand streight. I would do some mirror training and make correct my form a lot more if this is the case.
Also when you round house kick dont kick with the shin, you should kick with either the ball of the foot (not as common more for tma guys) or with the shin. You kick with the shin not the instep. If you blast to hard with the instep you will hurt yourself.
I would address all of these issues with your instructor and he can help you out by showing you the accurate ways of doing these things and point out what your doing wrong.
