It is a zombie survival simulation. It is still in Alpha so some things are still a but buggy but it is expected if you purchase a game that is still in Alpha.
Basically you begin with the clothes on your back and a flashlight. You pretty much just explore the map (which is quite large) scavenging for things to survive while trying to stay alive with zombies and other players running around. Some players are helpful, some don't care about you and some just kill you on sight. If you die, you start from scratch.
You have to find things like water, soda, food, etc... to keep your character going. There are weapons in the game although they are difficult to find. Different things like helmets and other clothing items that help you defend yourself.
The landscape is as if everyone has already left all of the towns. Houses and buildings look rundown and looted, no power, no nothing. So it gives you a weird feeling when you are going through the towns. Like you and the other players really are the only survivors.
You can use your mic to talk in the game world and it works well. (The further away you are, the harder it is to hear each other)
The system to keep someone alive is awesome as well. You can find things like bandages (or rip clothing into bandages), IV kits, bloodbags, syringes, vitamins, antibiotics, ephedrine, morphine, etc... that can all help you revive yourself or a friend or just stay alive. You can transfuse blood if you have the correct items to do so but the blood types have to match.
They intend on adding vehicles and other things to the game as they progress.
